Kyrle's disease

This is a perforating dermatosis characterized by keratotic plugs within crater-like papules, a different morphology than the excoriated, dome-shaped nodules of prurigo nodularis.

Lymphomatoid papulosis

This is a low-grade lymphoma characterized by recurrent crops of papules and nodules that spontaneously ulcerate and heal with scarring.

Multicentric reticulohistiocytosis

This is a systemic disease with destructive arthritis and reddish-brown "coral bead" nodules, not the intensely itchy, excoriated nodules of prurigo.

Nodular amyloidosis

These are waxy, non-pruritic nodules composed of amyloid deposits, whereas prurigo nodules are intensely itchy and are a reactive process.

Pemphigoid nodularis

This is a variant of bullous pemphigoid where the intense itch leads to the formation of prurigo nodules; it is distinguished by immunofluorescence showing antibodies characteristic of pemphigoid.

Polyarteritis nodosa

Cutaneous PAN presents as tender, subcutaneous nodules along the path of arteries, often with livedo reticularis, a different presentation and pathology.

Viral warts

A wart is a verrucous papule caused by HPV, not an intensely itchy, excoriated nodule.
