Lichen nitidus

This is characterized by pinpoint, shiny, flat-topped papules, which are much smaller than the classic polygonal papules of lichen planus.

Lichen simplex

This is a secondary phenomenon, a single leathery plaque created by chronic scratching, not a primary eruption of multiple papules like lichen planus.

Lichenoid drug reaction

This can be clinically and histologically identical to lichen planus; the key differentiator is the history of a new medication and the resolution of the rash upon its cessation.

Nodular prurigo

These are intensely itchy, excoriated nodules, a different morphology than the flat-topped papules of lichen planus.

Scabies

Scabies is an infestation characterized by burrows and intense nocturnal itch; while it can trigger a lichenoid reaction, the primary findings are different.
